Your relationship with Neighborhood Health Services (NHS) begins with these few simple steps. Please take a moment to review this section to answer any questions you may have.
Make an appointment
To make an appointment, simply call our clinic at 850-224-2469. Please have the following information available to schedule an appointment or procedure:
- full patient name
- date of birth
- which provider they will need to see
- the nature of the patient’s visit.
New Patients
If you are a new patient you can save time by completing our New Patient Form prior to your visit. You may download this form from our Forms page.
For your first visit, please come 20 minutes early and bring:
- Proof of identification. This can be a driver’s license, a social security card, a birth certificate, or another form of photo ID.
- Proof of Residency. For example: a current driver’s license, utility bill, property tax statement, or a voter’s registration card.
- Proof of Income. For example: a check stub or a letter from your employer. If you have no income, you may provide a notarized letter, signed by the person who is providing you assistance.
Please remember to bring identification and verification as outlined in our FAQ section of this website.
Walk-ins

*Other Information *
If you must cancel an existing appointment, please call at least 24 hours prior to the scheduled time of your appointment. A “no-show” occurs when a patient fails to show for a scheduled appointment or cancels without 24 hours’ notice. No-shows may be subject to a fee.
